What the chart records
Intellicore charts the full permanent dentition in FDI notation on photographic tooth imagery — not abstract diagrams. Per surface, it records pathology and restorations with materials; per tooth, crowns and bridge units, root treatments, implants and missing teeth. Charting works from the patient record or chairside during a visit, in a dedicated operatory display mode.
Because the chart drives treatment planning, what you plan is attached to what you see: proposed work per tooth with prerequisite chains — extraction → implant → implant crown — through draft, approved and completed states, with what-if projection onto the chart.
Why event-sourcing matters clinically
Every charting entry is an immutable, dated record; the chart you see is derived from the full history. Nothing can be silently overwritten — corrections are new entries, and the original stays visible. When a clinical record is ever questioned, the history answers by construction, not by policy.
Treatments only complete through a visit's recorded outcomes, so the chart always reflects what actually happened in the chair — and selected plan items compose straight into ADA-coded, GST-compliant quotes and invoices.
